Approved assessors sign in with a one-time code sent to their email — no password to remember. Inside the tool you can also print an eight-page paper interview worksheet for running the conversation during a home visit. Your answers stay in your browser and are never sent to this site. Who this is for
The assessment is built for HES assessors doing a real technology evaluation for a specific person.
How to request access
Access is currently limited to individually approved assessors while the tool is reviewed with stakeholders. To request access, email hello@homeenablingsupportsny.org with:
- your name and work email address (the address you'll sign in with),
- your agency or organization, and
- your role and how you plan to use the assessment.
Approved emails are added to the sign-in list, and you'll get a confirmation. There is no cost. Each assessor is approved individually by work email address.
